Why do new varifocals never help for long?

Close-up of an eye with a patterned iris
In brief

What
A multifocal lens combines distance, intermediate and near in one artificial lens and replaces your own lens in a lens exchange.
For whom
From about 50 to 55 years with presbyopia or beginning lens clouding; a clear wish for freedom from glasses, good tear film.
Trade-off
More freedom from glasses than EDOF, but more often halos at night and slightly less contrast; adaptation 2 to 12 weeks.
Price
€3,500 per eye, fixed price for every lens type (monofocal, toric, EDOF, multifocal), incl. follow-up checks under the quality scheme. From €68/month.
Not suitable
Night drivers with high glare sensitivity (rather EDOF), untreated dry eyes, retinal disease, high short-sightedness under 55.

Varifocal glasses force you, without you noticing, into a rigid pattern of behaviour. Whether you have to lower your head unnaturally on the stairs down to the underground to avoid the dangerous “swimming” effect, or develop chronic neck tension from working at a computer in the office: the glasses are a physical and psychological barrier.

Multifocal lenses break through this barrier directly inside the eye. Our goal is to give you back visual spontaneity: letting your gaze glide smoothly from the horizon on the golf course to the smartwatch on your wrist, without reaching for an optical aid.

What is a multifocal lens?

A multifocal lens is an artificial lens that permanently replaces your own natural lens. A standard lens focuses only one distance sharply, usually the far distance, and reading glasses remain for everything close. A multifocal lens, by contrast, focuses light at several focal points; a trifocal lens at three: distance, screen distance and near. The procedure is performed as an outpatient under local anaesthesia and takes about ten to fifteen minutes per eye.

Because your own lens is removed, a cataract can never develop later: the treatment corrects presbyopia and pre-empts cataract surgery. If the capsule clouds over years later, that is a posterior capsule opacification and is cleared with a laser in a few minutes.

The multifocal lens differs from EDOF lenses in one respect: EDOF creates a continuous zone of sharpness instead of fixed focal points. That means fewer light phenomena at night, but for very small print a light pair of reading glasses is needed more often.

Technology

Which multifocal lenses do we use in Vienna?

The technology has developed rapidly. At EyeLaser Vienna we no longer implant outdated bifocal lenses (with only 2 focal points). Independently of any manufacturer, we draw on the absolute world elite of the latest lens generation:

Smiling patient lying under a surgical microscope, with a masked doctor beside her
  • The new generation of trifocal lenses (focus: absolute near vision): these diffractive lenses focus light at three exact focal points: distance, intermediate (computer) and absolute near (fine print). The latest models have reduced night-time rings of light (halos) to a minimum and maximised contrast.
(Our premium selection: enVista ENVY by Bausch + Lomb, PanOptix® Pro by Alcon, ODYSSEE by J&J.)
  • The refractive EDOF lenses (focus: dynamics and night vision): “Extended Depth of Focus” offers a stepless visual channel from distance to smartphone, often with a smooth design without rings. The huge advantage: brilliant contrast and almost zero halos at night. Ideal for frequent drivers.
(Our premium selection: PureSee™ by J&J, LuxLife by B&L.)

Suitability

Am I suitable for a multifocal lens?

A first orientation. The binding decision is always made only after the full preliminary examination.

🟢  GREEN LIGHT

Ideal conditions. Age from about 50 to 55 with pronounced presbyopia · long-sightedness with reading problems · first signs of natural lens clouding · good lighting in your working day and the clear wish to be free of glasses.
🟡  AMBER LIGHT

Careful medical clarification needed. Dry eyes (MGD): a broken tear film noticeably amplifies halos. It must be treated before the measurement · frequent night drivers: we check whether EDOF lenses are the safer choice · previous LASIK: requires more complex special lens calculations.
🔴  RED LIGHT

No multifocal lens. High short-sightedness under the age of 55 to 60 because of the risk of retinal detachment · advanced macular degeneration or advanced glaucoma with visual field loss. Here the diffractive lens further reduces the already weakened contrast; we then advise a standard monofocal lens.

We use the most innovative lenses in the world, such as the enVista ENVY or PanOptix Pro. Nevertheless, one thing is particularly important to me in the consultation: I do not make unrealistic promises, but explain the physics of light and the compromise of neuroadaptation transparently. Only if you understand these processes will you be satisfied with the result for a lifetime.”

Costs

What does a multifocal lens cost in Vienna?

FIXED PRICE · EVERY LENS

Multifocal lens, like every other lens
€3,500 per eye
From €68 per month.
What you should know
✓Care package after the treatment: you take your eye drops for the first days home with you directly from us.
✓No surcharge for the premium lens. Monofocal, monofocal-plus, toric, EDOF or multifocal. Every lens costs €3,500 per eye.
iYou pay for the clinical process, not for the lens: diagnostics, calculation and selection of the lens, surgery and aftercare. Which lens is implanted is decided by the measurements; so there is no incentive to recommend a more expensive one.
iThe Austrian public insurance (ÖGK) pays for cataract surgery with a standard lens in a public hospital: there, however, without free choice of surgeon. With us you are a private patient and pay one all-inclusive price; nothing is added on top of an insurance benefit.
✓Payment in instalments is possible.

Lens surgery including the lens, per eye: any lens type €3,500
When we say no: with diseases of the retina or macula we do not implant a multifocal lens; it needs a healthy eye to play to its strengths. Pronounced dry eye we treat first, otherwise image quality suffers. And anyone who drives a lot at night for work and has the highest contrast demands is more likely to be recommended an EDOF or monofocal solution by us, including an honest explanation why.

How does multifocal lens surgery work?

Step 1Free suitability test. About 15 minutes, no obligation.
Step 2Detailed preliminary examination. Biometry, corneal topography, retinal findings and the alpha distance. Only this measurement decides whether a multifocal lens makes sense for your eye at all.
Step 3The procedure. About 15 minutes per eye, under anaesthetic drops, painless. Your own lens is removed and the folded artificial lens is inserted.
Step 4Check-up the next day, then according to the schedule. The second eye is usually operated on in the same session, or on a separate day on request.
Step 5Neuroadaptation over three to six months. The brain learns to switch between the planes of sharpness.

How does healing progress, and when will I see really well?

Most people already see well enough for everyday life the day after the procedure. The real adjustment then happens in the head: a multifocal lens delivers several images to the brain at once, and it has to learn to pick the sharp one each time.

  • Week 1: no rubbing, no heavy lifting, no swimming pool and no sauna.
  • Weeks 2 to 4: reading without glasses becomes more natural day by day. Reading glasses as a safety net are normal in this phase.
  • Months 2 to 3: rings of light at night fade into the background.
  • Month 6: the result is final for almost all patients.

What are the risks of a multifocal lens?

iRings of light and glare at night. Present in almost everyone at first, no longer disturbing for most after three to six months, but not completely gone for everyone.
iSlightly lower contrast than with a monofocal lens. That is the price for light being distributed across several planes.
iThe usual risks of intraocular surgery: infection, macular oedema, retinal detachment. All rare.
iPosterior capsule opacification, treatable painlessly with the YAG laser.

That is why with us the measurement decides, not the wish: anyone who drives a lot at night or whose alpha distance is too small is recommended an EDOF lens or a toric monofocal-plus lens, at the same price.

Honesty

What optical compromise does a multifocal lens involve?

In leading international clinics, patients are told a physical truth that we also live by uncompromisingly at EyeLaser Vienna: every multifocal lens splits the light. Nature cannot be tricked. There is a difference between “sharpness” (quantity) and “contrast” (quality).

The MTF principle (Modulation Transfer Function): so that you can see a book and the mountains on the horizon sharply at the same time, diffractive (trifocal) lenses have to split the incoming light.

Smiling older man at an eye examination, the doctor's hand on his shoulder
  • The physical fact: through this splitting the eye loses about 10 to 15 % of the light energy for each focal point. You can read tiny letters (high sharpness), but the contrast (the ability to distinguish fine shades of grey in twilight) drops minimally.
  • What this means for your everyday life: in a dimly lit restaurant you sometimes need a little more light to read the menu (phone torch) than a healthy 20-year-old. In addition, slight rings of light (halos) arise at the optical grooves at night.
  • The neuroplasticity of the brain: in the first 3 to 6 months after surgery your brain learns to filter these new optical signals and largely blank out halos (neuroadaptation).

Our promise: through our “visual lifestyle mapping” at the preliminary examination we find exactly the optical compromise that feels like a gain for you.

Precision

How precisely is the lens calculated for my eye?

A multifocal lens forgives no mistakes. If it sits decentred by just a fraction of a millimetre, or if the cornea is rough, the optical side effects multiply enormously.

Person in a white coat pointing at a cross-section model of the eye
  • Ocular surface optimisation: a multifocal lens only works perfectly if your tear film is intact. Dry eyes scatter the light and falsify the AI-supported measurement. We treat dry eyes as a matter of course before biometry (for example with IPL light therapy) to create a perfect basis.
  • AI biometry (MS-39 AXL): we measure your eye with high-resolution laser technology. The data is run through the most modern artificial-intelligence formulas (such as the Kane formula or Hill-RBF). The AI analyses thousands of parameters of your eye to predict the power of the artificial lens to the micrometre.
  • Ultrasound through a small incision: we remove your natural lens with ultrasound (phacoemulsification) through an incision of 2.2 to 2.4 mm. A femtosecond laser (FLACS) is not used: according to the ESCRS it is equivalent to the manual technique and does not measurably improve the visual acuity achieved.

Safety

Why do we often turn away short-sighted patients from 45?

In many places, a multifocal lens is recommended across the board from the age of 45. We at EyeLaser Vienna do not do that.

Thoughtful young woman with her hand on her chin against a light grey background

If you are highly short-sighted, your eyeball has grown anatomically too long. Your retina is under mechanical tension. If we now remove your voluminous natural lens and replace it with a very flat artificial lens, the vitreous body moves inside the eye.

The consequence: in short-sighted people this massively increases the risk of a dangerous retinal detachment (ablatio retinae)!

If you are highly short-sighted and younger than 55 to 60, we usually decline lens exchange for your safety. We then evaluate gentler alternatives such as implantable contact lenses (EVO ICL) or the PresbyMAX laser.

Frequently asked questions (FAQ) about multifocal lenses in Vienna

What is the difference between multifocal, trifocal and EDOF lenses?

“Multifocal lens” is the general umbrella term for all multi-power lenses. Trifocal lenses (e.g. enVista ENVY, PanOptix Pro, ODYSSEE) are a newer subtype: they have three fixed focal points (distance, computer, near). EDOF lenses (e.g. PureSee, LuxLife) have a smooth design without rings. They offer good vision at distance and at the computer with clearly fewer halos at night, but for the finest print you may need a light pair of reading glasses.

Is the procedure painful?

No. Your eyes are completely numbed locally with drops. During the operation (which takes only 10 to 15 minutes per eye) you feel only slight pressure, but no pain at all.

Will I feel the multifocal lens in my eye?

Absolutely not. The premium lens is placed in the capsular bag of your eye with micrometre precision. Because there are no pain nerves inside the eye, you feel the implant neither when blinking nor when rubbing.

How long does the artificial lens last?

The biocompatible high-performance acrylates do not decompose and do not cloud over. The multifocal lens lasts your entire life and never needs to be replaced.

Can I still get a cataract after lens exchange?

No! Because your natural lens, which is prone to clouding, has been removed and replaced with a crystal-clear artificial lens, you can never develop a cataract again for the rest of your life.

What is a posterior capsule opacification and how is it treated?

Months or years after surgery, the body’s own tissue capsule in which the new lens sits can become slightly cloudy (posterior capsule opacification). Vision becomes milky. We “polish” this capsule painlessly in our practice with a YAG laser in 60 seconds. After that, vision is crystal clear for good.

What is neuroadaptation?

Because diffractive multifocal lenses project several images (distance, near) onto the retina at the same time, your brain has to learn through neural plasticity to focus on the image currently needed and blank out the others. This fascinating neurological learning process usually takes 3 to 6 months.

What are halos and do they disappear again?

Halos are fine circles of light that appear at night around bright light sources (e.g. car headlights) because the light is refracted at the grooves of the lens. Through neuroadaptation the brain learns to blank out these halos. For most patients they are no longer disturbing in everyday life after a few months.

Why do I sometimes need more light for reading?

Because the light is physically split into several focal points, not 100 % of the light energy reaches the retina for the near image (physical contrast loss of about 10 to 15 %). In a dimly lit restaurant a little more light (phone torch) therefore often helps to read the menu razor-sharp.

When will I see sharply after the operation?

Visual recovery is rapid. As early as the first day after surgery most patients see excellently in the distance. The fine-tuning of near vision stabilises over the following weeks.

When can I work again?

Most patients are fit for work again 2 to 3 days after the operation and can work at a screen. You should avoid heavy physical work for 1 week.

How long do I have to wait for sport?

Light sport (jogging, cycling) is allowed after 3 to 4 days. Swimming (lakes, thermal baths) and sauna are strictly forbidden for 3 weeks to avoid serious bacterial infections inside the eye (endophthalmitis).

May I drive myself after the operation?

Absolutely not on the day of surgery. At the medical check-up the next day Dr. Derhartunian gives you clearance as soon as the legally required visual acuity has been reached.

Is a femtosecond laser (FLACS) used?

No. We remove the natural lens with ultrasound (phacoemulsification) through an incision of 2.2 to 2.4 mm. According to the recommendations of the European Society of Cataract and Refractive Surgeons (ESCRS), laser-assisted surgery (FLACS) is equivalent to the manual technique and does not measurably improve the visual acuity achieved. We therefore do not offer it, and there is no femtosecond laser surcharge.

Why must I not use face cream or make-up on the day of surgery?

Make-up particles and cream vapours endanger the absolute microbiological sterility in the operating theatre. Come with a freshly washed, “bare” face.

May I wear make-up after the procedure?

Please refrain completely from eye make-up for at least 1 week to prevent dangerous infections from bacteria in mascara.

Why must I not wear contact lenses before the preliminary examination?

Contact lenses deform the cornea and falsify the AI-supported biometry values we need to calculate your multifocal lens to the micrometre. Leave soft lenses out for 1 week and hard lenses for 4 weeks before the examination.

Are both eyes operated on the same day?

Both eyes are usually operated on in the same session, each as a separate procedure with its own instruments. On request, we operate on them on separate days.

Can I be operated on despite astigmatism?

Yes, absolutely. We order the multifocal lenses in a special “toric” version. These are precisely adapted to your axis and correct presbyopia and astigmatism at the same time.

Is blindness possible as a result of the procedure?

The risk of a serious complication up to blindness is extremely low (well below 1 in 10,000). The greatest theoretical danger is an infection. We minimise this risk uncompromisingly through highly sterile surgical conditions (class 1 operating theatre) and strict hygiene protocols.

Why do you often turn away short-sighted people under 60?

Short-sighted eyes are anatomically longer, the retina is under mechanical tension. If we remove the body’s own voluminous lens and insert a flat artificial lens, the vitreous body moves. The risk of a retinal tear or retinal detachment rises significantly. We do not expose you to this risk unnecessarily (especially before the age of 60).

Why is treating dry eyes BEFORE surgery so important?

The tear film is the very first refractive layer of the eye. If it is rough and broken, light scatters on the cornea, which massively worsens the halos of the multifocal lens and falsifies the AI measurement. We treat dry eyes in advance (ocular surface optimisation).

What does "mix and match" (blended vision) mean?

With mix and match, each eye receives a different type of lens. We often place an EDOF lens with few halos (e.g. PureSee) in the dominant eye, for distance, intermediate vision and night driving, and a trifocal lens in the non-dominant eye, for fine reading. Which trifocal lens fits is decided on the basis of your measurements. The brain combines both visual impressions. Whether mix and match is an option for you depends, among other things, on angle alpha and angle kappa and on how regular your cornea is. We measure both during the pre-operative examination.

Why does the preliminary examination at EyeLaser take almost two hours?

Premium safety tolerates no haste. The optical AI measurement (MS-39 AXL), the pupil dynamics analysis (pupillometry), the endothelial cell count and the retinal check with dilated pupils (cycloplegia) take time. Added to this is the detailed consultation about the physical compromise of the lens.

How much do multifocal lenses cost in Vienna?

At EyeLaser, lens surgery costs €3,500 per eye, regardless of which lens is implanted. A monofocal, monofocal-plus, toric, EDOF or multifocal lens costs exactly the same.

With us you do not pay for the lens model but for the entire clinical process: the diagnostics, the calculation and selection of the lens, the surgery and the aftercare. There is a deliberate reason for this: because the choice of lens does not change the price, it is made purely on medical grounds and never for commercial reasons. The femtosecond laser can be added as an option (€1,000).

Does the Austrian health insurance (ÖGK) cover the costs?

No. The public insurers pay only for a simple standard lens in cases of medically pronounced cataract. Lens exchange with multifocal lenses for freedom from glasses is considered a private service. You can, however, deduct the costs in your Austrian tax return.

Do you offer payment in instalments?

Yes, in cooperation with our reliable financing partners we offer flexible instalment models.

Can it happen that the dioptres are not hit 100 %?

Yes. Because every body heals individually, in rare cases (about 2 to 5 %) a small residual error remains. This is where our EyeLaser quality promise applies: we can usually perfect the optical result without any problem through a fine laser touch-up on the cornea (e.g. with Trans-PRK).
