How does ReLEx SMILE work?

With ReLEx SMILE, the eye is first locally anesthetized with eye drops. The femtosecond laser creates the lenticule (a thin corneal lens in the intact cornea) inside the cornea. The lenticule is removed through a three to four millimeter incision. This corrects the visual defect and significantly improves the patient’s vision. With Relex Smile, there is no flap, which reduces the risk of treatment.

The protective corneal surface layer and the upper corneal layer (the epithelium) remain untouched, which maintains the stability of the cornea.

ReLEx SMILE treatment procedure

You have already seen how lenticule extraction with the ReLEx SMILE method works. The treatment only takes a few minutes and is painless thanks to local anesthesia. The following pre- and post-treatment steps are part of the ReLEx SMILE treatment procedure:

Before ReLEx SMILE (Pro) lenticule extraction

Before treatment can begin, a detailed preliminary examination is essential. Your eyes will be comprehensively examined and it will be determined whether you are suitable for ReLEx SMILE. Various diagnostic devices are used for this. The data collected serves as the basis for the subsequent treatment.

Healing process with ReLEx SMILE

Immediately after ReLEx SMILE, it is normal for the eyes to water and/or itch. The eyesight is not yet at the level it will reach later. The attending doctor may prescribe medication to support healing and relieve pain after the operation.

Vision recovers within a few hours to days after ReLEx SMILE. For a smooth recovery, it is normal to have at least two follow-up appointments. In the first few days, vision feels unfamiliar, visual acuity changes slightly and the brain has to adjust to the changes. After four to six weeks, the adjustment is complete and maximum vision is achieved.

The Zeiss VISUMAX 800 is the latest femtosecond laser and the successor to the VISUMAX 500. The new design makes the treatment even more comfortable and ensures a fast, painless and robotically assisted procedure.

Risks of the ReLEx SMILE method?

The ReLEx SMILE method is considered to be extremely safe and low-risk. However, like any medical procedure, ReLEx SMILE is associated with certain risks and complications can occur. The risk of over- or undercorrection is minimal. If vision deteriorates again after ReLEx SMILE, conventional correction with the Femto, LASIK or PRK procedure is usually the only option.

  • Minor signs of abrasion and tearing at the incision
  • Reduced vision in poor lighting conditions (in the evening and at night)
  • Occasionally a feeling as if there is a foreign body in the eye, dryness of the eye
  • Surgical complications associated with the removal of the lenticule

As no flap develops with ReLEx SMILE, typical flap-related complications cannot occur. In most cases, there are no complications and if they do occur, they are often temporary and can be treated.
