Escaping the Progressive Lens Trap: Why New Glasses Are Not the Goal

Many patients come to us because they can no longer tolerate the side effects of their expensive progressive lenses in everyday life:

Augenlasern in Wien
  • The Neck Syndrome (Cervical Spine): To see clearly on the PC, you have to hold your head at unnatural angles. Chronic tension is the result.
  • The “Swimming Effect”: When descending stairs, the steps blur, you feel insecure and have to lower your head unnaturally.
  • Sports Restrictions: When golfing, playing tennis, or skiing, the rigid viewing zones of the glasses massively interfere with hand-eye coordination.

We don’t prescribe glasses that you have to “get used to.” We reconstruct the optical dynamics directly in your eye.

The Lens Revolution 2026: We Implant the World Elite

The market for artificial lenses is developing rapidly. What was standard yesterday is often outdated today. At EyeLaser Vienna, we are completely manufacturer-independent. We divide the currently most modern premium lenses into two categories:

Augenlaser

1. The New Generation of EDOF Lenses (Continuous & Halo-Free)

EDOF (Extended Depth of Focus) means “extended depth of field.” The very latest models (such as the PureSee™ from Johnson & Johnson or the LuxLife from Bausch & Lomb) operate on a purely refractive principle without optical rings.

  • Your Advantage: The corneal surface of the lens is continuously smooth. This means that light is not scattered. You experience brilliant vision from a distance to your smartphone – completely without light rings (halos) at night and with maximum contrast. Perfect for people who drive a lot at night. For extremely small print (package inserts), light reading glasses may be needed.

2. Diffractive Trifocal Lenses (Maximum Glasses-Free Vision)

Trifocal lenses bundle light through tiny optical rings (diffractive principle) into three exact focal points: distance, intermediate range (PC), and absolute near (reading tiny print).

  • Our Premium Selection: We rely on the absolute flagships of the industry, such as the innovative ODYSSEE (Johnson & Johnson), the ENVY Envista (Bausch & Lomb) or the proven PanOptix® Plus (Alcon). This latest generation offers superior contrast and has massively reduced nocturnal halos compared to older models.

The Physics of Vision: The Honest Optical Compromise

In leading international clinics, patients are taught a physical truth that we also live by at EyeLaser Vienna: Nature cannot be tricked. Every premium lens is an optical compromise. The art is to find the compromise that perfectly suits your life.

Grauer Star (Katarakt) Untersuchung
  • The Physics of the Trifocal Lens: To enable you to read the finest print without reading glasses, the lens must divide the incoming light into three focal points. This division causes the eye to lose approx. 10 to 15% of light energy (loss of contrast). In addition, slight light circles (halos) appear at the grooves at night. The compromise: absolute freedom from glasses versus slight optical phenomena in the dark.
  • The Physics of the Refractive EDOF Lens: Here, light is hardly divided. Almost 100% of the light energy reaches the retina. The contrast is brilliant, halos are practically non-existent. The compromise: the depth of field does not extend quite as close as with the trifocal lens; for threading a needle, you need good light or reading glasses.

Together with Dr. Derhartunian, after extensive diagnostics, we select precisely the physical profile that matches your everyday life.

Medical Masterclass: FLACS & AI-Supported Calculation

Even the best lens in the world only works if it is seated in your eye with micrometer precision.

Tropfanästhesie oder Dämmerschlaf bei der Augenoperation
  • AI Biometry: We use state-of-the-art formulas and algorithms developed in leading clinics in the USA and Asia. Artificial intelligence analyzes thousands of parameters of your eye to predict the perfect diopter number for the artificial lens.
  • Scalpel-Free with the Femtosecond Laser (FLACS): At EyeLaser Vienna, we offer the procedure as Femtosecond Laser-Assisted Cataract Surgery. The laser opens the lens capsule in a mathematically perfect circle (capsulorhexis). The result: the premium lens is perfectly centered – the decisive factor for flawless optics.

Safety First: Why We Often Decline Myopic Patients Under 45

Many clinics operate on every patient over 45 with a lens exchange to make a profit. We do not.

Femto-LASIK

If you are severely myopic, your eyeball has grown anatomically too long. Your retina is under tension. If we now remove your large natural lens and replace it with a very flat artificial lens, the vitreous body moves.
The Medical Consequence: In myopic patients, this massively increases the risk of a dangerous retinal detachment (ablatio retinae)!

If you are myopic and younger than 55-60 years, we usually decline lens exchange for your safety. We then evaluate gentler alternatives, such as implantable contact lenses (EVO ICL) or laser procedures.

The EyeLaser Eligibility Check: Traffic Light System for Lens Exchange

EDOF-Linsen

🟢 Green Light (Ideal Conditions):

  • Age from approx. 50-55 years with presbyopia.
  • Farsightedness (Hyperopia) with onset of reading problems.
  • First signs of natural lens opacity (cataract).

🟡 Yellow Light (Precise Medical Clarification Needed):

  • Dry eyes: The tear film must be treated before a procedure, as it is crucial for sharp vision with premium lenses.
  • Previous laser surgery (LASIK) in youth: Requires our special AI calculations for the new artificial lens.

🔴 Red Light (Strict Rejection for RLE with Premium Lenses):

  • Severe myopia under 55-60 years (Due to massive risk of retinal detachment).
  • Advanced macular degeneration (AMD) or advanced glaucoma. (Here, from a medical point of view, surgery with standard monofocal lenses is usually the only sensible option).
Dr Victor Derhartunian

“In refractive lens surgery, there is no standard solution. We implant an optical system with which you will spend the next 30 or 40 years of your life. That is why we work independently of manufacturers and rely on the most innovative lenses in the world – such as the PureSee or ODYSSEE. At the same time, we have the medical courage to show you the physical limits and to honestly decline you in cases of retinal risks. Trust is built on professional competence and absolute transparency. We measure ourselves by this every day.”

Frequently Asked Questions (FAQ) about Lens Exchange in Vienna

What is the difference between refractive lens exchange and cataract surgery?

Surgically, it is the same procedure. Standard cataract surgery is performed when the lens is already severely clouded and uses a simple standard lens (reading glasses remain necessary). Refractive lens exchange uses high-tech lenses (EDOF/Trifocal) for a life largely without glasses.

What is the difference between trifocal and EDOF lenses?

Trifocal lenses (like ODYSSEE, PanOptix Plus, ENVY) use fine grooves to divide light into three focal points. They offer maximum freedom from glasses even for the tiniest print, but can cause slight halos at night. Refractive EDOF lenses (like PureSee, LuxLife) use a smooth principle. They offer excellent vision at a distance and on the PC without halos at night. For extremely fine print, reading glasses may be needed.

Is the procedure painful?

No. Your eyes are completely numbed locally with drops. You will only feel slight pressure during the surgery (10 to 15 minutes per eye).

Will I feel the new lens in my eye?

Absolutely not. The premium lens is inserted with micrometer precision into the capsular bag of your eye. There are no pain nerves inside the eye.

How long does the artificial lens last?

The biocompatible acrylate materials do not decompose. The lens lasts your entire life and does not need to be replaced.

Can I still get cataracts after a lens exchange?

No! Since your natural lens, which is prone to clouding, has been replaced by a crystal-clear artificial lens, you can never again suffer from cataracts (cataract) in your lifetime.

What is a secondary cataract and how is it treated?

Months or years after lens exchange, the body’s own tissue capsule, in which the new lens sits, can become slightly cloudy (secondary cataract). This is a normal cellular reaction. We “polish” this capsule painlessly with a YAG laser in 60 seconds. After that, vision is crystal clear forever.

What is neuroadaptation?

Your brain must learn to perfectly filter the new, high-resolution optical signal from the premium lens and to suppress any nocturnal light effects. This learning process (neuroadaptation) usually takes 3 to 6 months.

When will my vision be sharp after the surgery?

Visual recovery is extremely fast. Most patients see excellently at a distance on the very first day after surgery. The fine-tuning of near vision stabilizes in the following weeks.

When can I return to work?

Most patients are able to return to work and work on a screen 2 to 3 days after the operation. Heavy physical work should be avoided for 1 week.

How long do I have to wait to exercise?

Light sports (jogging) are permitted after 3 to 4 days. Swimming (Alte Donau, thermal baths) and sauna are strictly forbidden for 3 weeks to avoid severe bacterial infections inside the eye (endophthalmitis).

Am I allowed to drive myself after the surgery?

Absolutely not on the day of the surgery. During the medical check-up on the following day, the doctor will give you permission as soon as the legally required visual acuity is reached.

What does FLACS mean?

FLACS (Femtosecond Laser-Assisted Cataract Surgery) is the premium method. A computer-controlled femtosecond laser opens the lens capsule in a mathematically perfect circle. This guarantees the exact centering of the new high-end lens, which is crucial, especially for trifocal and EDOF lenses.

Why can't I use face cream or make-up on the day of the surgery?

Make-up particles and cream vapors massively endanger microbiological sterility in the operating room.

Can I wear make-up after the procedure?

Please completely refrain from eye make-up for at least 1 week to prevent infections from bacteria in mascara or eyeshadow.

Why can't I wear contact lenses before the preliminary examination?

Contact lenses deform the cornea and influence the AI-supported biometry values that we need to calculate your lens with micrometer precision. Remove soft lenses 1 week and hard lenses 4 weeks before the examination.

Are both eyes operated on the same day?

For lens exchange, for safety reasons, we often prefer a gap of a few days to a week between the right and left eye to optimally monitor wound healing.

Can I be operated on despite astigmatism?

Yes, absolutely. We order the premium lenses (whether EDOF or trifocal) in the “toric” version. These are precisely adapted to your axis and correct presbyopia and astigmatism simultaneously.

What happens if a toric lens rotates in the eye?

In extremely rare cases (approx. 1-3%), the lens can rotate in the eye in the first few weeks, leading to blurred vision. In this case, we simply readjust the lens to the perfect position in a short sterile procedure.

Is blindness possible due to the procedure?

The risk of a severe complication leading to blindness is extremely low (far below 1:10,000). We minimize this risk through highly sterile operating conditions (Class 1 operating room), strict hygiene, and antibiotic drops.

Why do you often decline myopic patients?

In curved, longer eyes, the retina is under tension. If we remove the body’s own large lens and insert a flat artificial lens, the vitreous body moves. The risk of retinal detachment increases significantly. We do not unnecessarily expose your retina to this risk (especially before the age of 55).

What happens with severely dry eyes?

The tear film is the very first refractive layer of the eye; it must be intact for a premium lens to function optimally. We analyze your tear film in detail beforehand and, if necessary, initiate therapy (e.g., IPL light therapy).

What does "Mix & Match" (Blended Vision) mean?

To utilize the perfect advantages of both lens systems, we often place a halo-free EDOF lens (e.g., PureSee) in the dominant eye (for perfect night vision) and a trifocal lens (e.g., ODYSSEE) in the non-dominant eye (for fine reading). The brain seamlessly blends both visual impressions.

Why does the preliminary examination at EyeLaser take almost two hours?

Premium safety tolerates no haste. The optical AI measurement, endothelial cell count, and retinal check with dilated pupils require time. In addition, there is the detailed consultation with the surgeon about the optical compromise.

How much does lens exchange cost in Vienna?

Refractive lens exchange with state-of-the-art premium lenses often costs between 3,000 and 4,500 Euros per eye. The price reflects the high-precision customisation, the use of the femtosecond laser (FLACS), and the extremely high material costs of the global market lenses.

Does the Austrian health insurance fund (ÖGK) cover the costs?

No. The health insurance funds only pay for a simple standard lens in cases of medically pronounced cataracts. Lens exchange for freedom from glasses is considered a private service. However, you can deduct the costs for tax purposes in your employee assessment.

Do you offer installment payments?

Yes, in cooperation with our reliable financing partners, we offer convenient installment payment models.

Can it happen that the diopters are not 100% accurate?

Yes. Since every body heals individually, in rare cases (approx. 2-5%), there may be a small residual refractive error. Here, our EyeLaser quality promise: We can usually perfect the result easily through a fine laser touch-up on the cornea.
